Diagnosis or cause of accident
The deceased was just knocking off work, and entered the cage with two others to go to the surface. On the way, Thomas was seen to fall. His mates grabbed hold of him, but failed to save him before his head, which protruded outside the cage, was knocked against the timber in the shaft and death immediately followed.
