Hanson was found unconscious and died at Royal Perth Hospital on September 18. The accident occurred when the mechanism used to open and close a tarpaulin cover on a triple road train trailer "failed" while he was in the process of closing the cover. The Department of Mines, Industry Regulation and Safety (DMIRS) said the operating switch for the tarpaulin cover was in a location that placed the driver "in the line of fire" when the tarpaulin tube failed.
