Diagnosis or cause of accident
Over-wound the cage, severing the steel cable which flew into the engine room, striking Berry, the engine driver, and killing him.
According to the Mines Dept Annual Report 1913, Berry had been subject to fainting spells but, following a holiday, had returned to work with a medical certificate. It appeared that he may have fainted again. The engine room temperature was 106F (41C).
