1977 Agnew nickel mine, WA. Fall Down Shaft. The men were in a kibble at the surface ready to be lowered underground when, due to a malfunction of the winding engine mechanism, the kibble was raised at excessive speed and crashed into a beam at the top of the skyshaft, causing the rope supporting the kibble to break and the kibble to plunge to the surface brace.
