Diagnosis or cause of accident
White, a contractor from New Zealand, fell to his death while working at considerable height. Conflicting reports said Mr White fell between 10 to 40 metres to his death.The Department of Mines and Petroleum, who is investigating the incident, said the contractor had entered a digester vessel that had no scaffolding and fell a "considerable distance" to the bottom of the vessel.
