Diagnosis or cause of accident
Two men were killed in an explosive accident while lighting fuses on the No 5 Level North drive face. It would appear that cartridges or master fuses were not used and the men had trouble in lighting wet fuses and stayed too long at the face after lighting the first fuse. DMP Annual report 1960.
