Diagnosis or cause of accident
It was reported to the deceased that the pump in the main shaft was not drawing well and that the water was rising. He, therefore, descended the shaft to the 175ft. level to ascertain the cause. Bailing was being carried out meanwhile by means of a heavy canvas and iron bucket which when full weighed about 8cwt [400kg]. By some means this bucket caught in one of the skids when within three feet of the top of the shaft. The sudden strain broke the handle and the bucket was precipitated down the shaft, striking the deceased in its descent and carrying him to the bottom into 80ft. of water.
