Name Of Mine On Which Last Employed
Commodore GM, Peak Hill, Meekatharra, Western Australia

Diagnosis or cause of accident
Hearing a stoppage of the machinery, he went from his house to the mine in his pajamas and overcoat. After helping to replace the belt, which had slipped off the driving wheel, he stood talking to one of the employees, with his back to the belt. A gust of wind blew the tail of his overcoat onto the belt, and before the machinery could be stopped, he was carried into the workings, being mutilated to such an extent that death was almost instantaneous.
