Diagnosis or cause of accident
The deceased was riding down the shaft on a "gig" or skeleton cage used in shaft repairing. There was no means of communication between the "gig" and the engine driver; it is supposed that the "gig" jammed in the shaft for a space and then fell-away, the jerk when the slack rope was brought up being responsible for the death of Mr. Trevaskis, whose head and one arm were badly gashed.
